It is obtained by killing Paracelsus. He is an elemental champion located near dungeon wrong in felucca (exit out via the path and you should see elementals).

but dont you need the skull to give to the elemental lord summoner to summon him

That's the elemental lord. Paracelsus is spawned the same way as other spawns like barracoon.

A creation of Brunus....hence the confusion .

But to put things a bit more clearly:

You must summon the champion "Paracelsus" by killing off enough of his minions. Just like Barracoon or Neira or Mephitis, he has an alter with red candles and once they are all filled with red candles, he'll pop out. Once you kill him, a random person will be awarded the skull (If it's just you doing the killing, you'll get the skull).

Once you have the skull, you hand it over to that quest giver near dungeon wrong. The skull will summon the Elemental Lord for you to kill.

Just a word of caution, that elemental can shoot arrows better then that elf guy who said "They're taking the hobbits to Izenguard!"

first of all the elemantal lord (that you summon witht he skull) a custom to this shard and are you saying it just shoots out arrows

Nah, he's custom and he has around 150k hp.
