Yes, it is possible to get the belt drop. The belt appears on the corpse of the champion once it dies, so it's first come first served on all those that did enough damage for loot rights.

The current chance of dropping is 5% with a 50/50 split. Half of the time it will be the sash, the other half of the time it will be the belt.
