Hes either in twisted weald or in citadel, I forget which is called which. But both are on AoV and active.

I don't think any have been completed though. There is not much benefit at this point.

Ilhenir the Stained is in the crypt of Bedlam and he IS active on AOV but is never attempted due to the fact that he is not the easiest champion/champion spawn to do and he does not drop powerscrolls or any other special items, similar to Serado in tokuno islands (But you can get tots there from normal monsters).

O rly, I never even knew. I remember people asking a while back and he wasn't in yet. Must have missed those additions.

They were in one of the news updates waaay back when. Twaulo and ilhenir both exist in their spots (You'll find an altar for each). They aren't worth doing on OSI or here, but I plan on changing that after the revamp. For the curious though, you can try challenging the champion.

I can't recall now if they give sashes, but if they do, then yes they drop. These champs were added in WAY at the beginning, like, 2007ish maybe early 2008. I'd have to check the code to be sure.
